What is Proctofoam-HC?

Proctofoam-HC is a topical treatment for haemorrhoids, also known as piles. It contains two active ingredients; a corticosteroid called hydrocortisone, and a local anaesthetic called pramoxine.

The medicine can be used to treat external and internal haemorrhoids, using the provided applicator.


What are haemorrhoids?

Haemorrhoids, colloquially known as piles, are swollen lumps which appear inside and around the anus.

They often develop when excessive pressure is exerted on the blood vessels which surround the rectum. Excessive pressure can be caused by the weight of the uterus or from excessive straining during defecation, though the latter is the most common attributer to the condition.

What causes haemorrhoids?

Common causes of haemorrhoids include:

  • A low-fibre diet
  • Chronic Diarrhoea
  • Constipation
  • Extended periods of sitting
  • Family history of haemorrhoids (hereditary)
  • Obesity
  • Pregnancy
  • Straining when lifting something heavy

In many cases, haemorrhoids are small, painless and clear up on their own. Some people may not even realise they have them. However, these lumps can grow and become painful, especially when passing stools.

What are the symptoms of haemorrhoids?

Symptoms of haemorrhoids can include:

  • A distended lump hanging from the sphincter
  • Bleeding when going to the toilet
  • Itching around the anus
  • Mucus discharge after defecation
  • Soreness, redness and swelling around your anus

If it is the first time you are experiencing haemorrhoids, speak to your doctor. They will be able to make a diagnosis based on the physical symptoms, also known as the clinical picture.


How does Proctofoam-HC work?

Proctofoam-HC contains a corticosteroid and local anaesthetic to treat the inflammation and discomfort of piles.

The corticosteroid, hydrocortisone, works by preventing cells from releasing inflammatory chemicals. This reduces the signs of redness and swelling to the applied area and relieves some discomfort.

The local anaesthetic, pramoxine, prevent pain signals from being relayed along the nerves. This relieves the pain and itching associated with haemorrhoids.


How do I use Proctofoam-HC?

Proctofoam-HC can be used for internal or external haemorrhoids. Always use Proctofoam as prescribed by your doctor.

Always remember to wash your hands before and after using Proctofoam-HC.

Before using Proctofoam-HC, shake the canister vigorously for 30 seconds.

For external use:

  1. Expel a small amount of Proctofoam-HC onto a tissue, pad or two fingers.
  2. Apply to the affected areas.

For internal use:

  1. Take the applicator and pull the plunger back until it stops at the line marked on the side.
  2. Holding the applicator with the nozzle pointing down, insert the canister of medicine into the tip of the applicator.
  3. Press down firmly on the top of the canister using two fingers.
  4. Press down on the top of the canister with your fingers. Fill the body tube a quarter of the way with foam.
  5. Lift the applicator from the canister and Wait a few seconds for the foam to stop expanding. Do not fill the applicator in one go.
  6. Repeat steps 4 and 5 until the foam reaches the foam line. This may take 2 to 4 presses for the foam to reach the indicated line.
  7. While standing or lying on your side, lift one leg and gently insert the applicator into the back passage.
  8. Push the plunger of the applicator down fully.

After using the provided applicator, take the applicator apart and wash it thoroughly.


Proctofoam-HC side effects and cautions

All medicines carry the risk of side effects, though not everyone gets them. Knowing the side effects associated with your medicine is important, as it helps to determine when changes should be reported to your doctor.

Serious side effects may include:

  • Anxiety
  • Confusion
  • Difficulty sleeping
  • Feeling depressed, including suicidal thoughts
  • Feeling high (mania)
  • Hallucinations
  • Memory loss
  • Mood swings
  • Paranoia

If you experience any of the above side effects, see your doctor straight away.

If you experience any of the side effects listed below, continue using Proctofoam-HC but inform your doctor.

Other side effects include:

  • Blurred vision
  • Body hair (women)
  • Burning sensation
  • Dusky complexion
  • Infection
  • Irregular periods
  • Itching
  • Purple patches on the skin
  • Skin thinning
  • Unexpected fattening of the face, neck and body

If you experience any side effects, including those not listed above or in the patient information leaflet, report them to your doctor as soon as possible. 

What should I do if I experience an allergic reaction?

If you experience the signs and symptoms of allergic reaction, immediately stop using Proctofoam-HC and seek immediate medical attention.

Symptoms of an allergic reaction include:

  • Chest tightness
  • Difficulty breathing
  • Difficulty swallowing
  • Dry, red, and cracked skin
  • Itchy, red, watering eyes
  • Rash or hives
  • Swelling of the face, lips, tongue or throat
  • Wheezing

Proctoform-HC contraindications

Proctofoam-HC is not suitable for everyone. In order to determine whether Proctofoam is suitable for you, speak to your GP.

Do not use Proctofoam-HC if you are allergic to any of the ingredients listed in the patient information leaflet.

Do not use Proctofoam-HC if you are suffering from a bacterial, viral, or fungal infection.

Before using Proctofoam-HC, speak to your doctor if any of the following apply to you:

  • You have bipolar disorder
  • You have had depression while using steroids before
  • You have had severe depression
  • You have bowel disease
  • You or your close family have any of these issues

Will Proctofoam-HC interact with any other medication I am taking?

Please inform our prescriber of all the medicines, including those with or without prescription, health and diet supplements, which you take so that our doctor can determine any risk of interaction. Your doctor may need to monitor you carefully if you are taking medicine for HIV, such as ritonavir or cobistat.

Do not use proctofoam for more than 7 days.

Can I take Proctofoam-HC while pregnant or breastfeeding?

If you are pregnant, intend on becoming pregnant, or breastfeeding, please inform our prescriber for advice before taking Proctofoam-HC.

What should I do if I use more Proctofoam-HC than I should?

If you take more Proctofoam-HC than you should, see your doctor as soon as possible.

Repeated overuse of the medicine can increase your risk of side effects, such as fattening of the face, next and boxy, body hair (women), a dusky complexion with purple patches and thinning of the skin.

What should I do if I forget to use Proctofoam-HC?

If you forget to use Proctofoam-HC when you should, skip the missed dose and continue taking Proctofoam as you normally wou

Do not use more of the medicine to make up for a missed dose.

Can I drink alcohol while using Proctofoam-HC?

While treating haemorrhoids, you should avoid alcoholic and caffeinated beverages because they can have a diuretic effect. This reduces the amount of fluid in your body, which can lead to constipation and make it more difficult to pass stools.
